For first time patients:

Intake forms take about 30 minutes to fill out before your first visit. Please come early to fill these out or pick them up prior to your visit. The first appointment is an hour and follow ups range from 15 minutes to an hour depending on the situation. Please bring any prescription drugs with you and any current supplements that you are taking.
